Northwestern Buddy Bags Enrollment Form

Buddy Bags is a voluntary program designed to provide food assistance to elementary school families and ease the financial burden associated with food costs. If you choose to participate, a “Buddy Bag” containing nutritious, non-perishable food will be sent home with your child every Friday. There will never be a charge for this food. To take advantage of this opportunity, please complete the form virtually, return a physical copy to school, or call 765-457-HELP to enroll over the phone. A United Way representative will contact you once your child is enrolled in the Buddy Bags program.

*Please note that students from the same family may each receive a Buddy Bag. A separate form must be filled out for each child.

By submitting this form, I understand that the food is being provided at no charge. I will not hold United Way Serving Howard and Tipton Counties nor the school corporation responsible for any negative incidents due to my child’s participation in this voluntary program. I understand that the likes, dislikes, and food allergies of those receiving food will not be monitored. If my child/family cannot use the provided food, we will pass it on.
